You can expect:
- 1-on-1 health coaching with a certified diabetes care and education specialist to help you with nutrition, physical activity, blood sugar, and everyday life with diabetes
- Medication support with a diabetes provider to make sure you have the right prescriptions to be at your best
This clinic might be right for you if you have:
- High average blood sugar (hemoglobin A1c)
- A hard time reaching your health goals on your own
- New health challenges

- Tips for checking your blood pressure — if you need a blood pressure cuff, the clinic will provide one at no cost
- Medication support from Devoted Medical pharmacists and nurse practitioners — they may suggest new medications or change your doses to get better results

Congestive Heart Failure Clinic
If you have congestive heart failure, this clinic offers extra support from a team of doctors, nurse practitioners, nurses, and pharmacists. You’ll get:
- 1-on-1 health coaching with a nurse who specializes in heart care. You’ll learn personalized insight and tips to help you stay on your medications, eat heart-healthy foods, and know what symptoms to watch for.
- Daily weight monitoring with a smart scale that automatically shares your weight with your team every day. This helps your team keep an eye out for big weight changes that may mean your heart failure is getting worse.
- Medication support based on the latest research, so you know you’re on the right medications for heart failure. As your medications or doses change, they’ll check in with you often to make sure everything is going okay.
Cardiology Clinic
You may qualify for this clinic if you need help managing a heart condition like:
- Arrhythmia (irregular heart beats)
- Coronary artery disease
- Heart failure
- Hypertension (high blood pressure)
- Peripheral arterial disease
The clinic team of doctors, nurse practitioners, social work case managers, pharmacists, and clinical assistants will partner closely with you to:

Chronic Kidney Disease Clinic
